Hilton Community Health Centre, Fremantle Community Health, forms part of the South Metropolitan Health Service. Community Health Nurses and Aboriginal Health workers deliver primary health care with a community development focus. The philosophy of the service is to promote health, prevent disease and disability utilising early intervention strategies such as screening and surveillance, health education and health promotion with a strong emphasis on encouraging personal responsibility for health.

Activities include:

  • Child health including a limited antenatal parenting home visiting service for clients with special needs through our Early Years Program.
  • Aboriginal Health - our Aboriginal staff provide a holistic approach to health education
  • Diabetes Education for people with Type 2 diabetes. Individual and group education by a diabetes nurse educator. Please note there is no dietitian available.
  • Enuresis program is available for children from 6-12 years old.
  • Immunisation Services for children
